Thank you for agreeing to take part in this Dwarf Sports Association UK (DSAuk) sport and activity survey. The survey will help DSAuk with its work across the UK. This is your chance to let us know which activities and sports you are currently taking part in and what you would like to try or do more of.   

 

The survey is designed to gain information from the current DSAuk membership as well as the wider dwarf/restricted growth population and should only take between 10 and 15 minutes to complete. The information you provide will be kept confidential in accordance with our Privacy Policy. The views collected in this survey will be used by DSAuk to provide evidence and support for their working practices.

 

At the end of the survey you will be given the option to enter into a prize draw for a £50 Amazon voucher.

If there is more than one person with dwarfism/restricted growth living at the same address, please ensure that a survey is filled out for each person. We want to gather as many people’s views as possible. 

If you are filling the questionnaire out on behalf of someone else, please complete all questions in the survey from the perspective of that person.
